Human Services Worker, Adult Disability Enrollment-Bilingual Preferred

Position Type:  Full-Time, Hourly 

Department:  Disability Resource Center (Marcia P. Coggs Location) 

Compensation Range:  $20.41 - $28.04 per hour
Closing Date: Monday, August 26th, 2019 @ 5:59 p.m. CST 

The mission of the Disability Resource Center (DRC) of Milwaukee County is to enhance the quality of life for all individuals with physical, sensory, and intellectual disabilities along with their support networks living in Milwaukee County. We do this by addressing their needs and providing Individualized opportunities for persons to participate in the community with dignity and respect, while acknowledging their cultural differences and values.

 

Job Summary: Under the direction of a DRC Supervisor, this position assesses and determines long-term care eligibility for customers who have a physical or intellectual/developmental disability. This HSW Enrollment Counselor will  provide options and enrollment counseling to customers; preferably to those who speak Spanish. The eligibility determination process involves completing a long-term care functional screens and calculating the results.

Job Responsibilities

  1. Completes a detailed assessment of client’s functional abilities utilizing state tools (long-term care functional screen) and calculates results. Consults with screen liaison and gathers medical documents as necessary.

  2. Reviews long-term care functional screen results and provides information to customers about long-term care options and resources available in Milwaukee County. Explains all forms, paperwork and signatures which are necessary for enrollment.

  3. Completes all necessary activities to complete and close case.

  4. Provides backup telephone coverage (Information and Assistance), regarding publicly funded long-term care options.

  5. Attends regular meetings with other members of their unit to discuss case and other issues.

  6. Attends regular trainings sponsored by Disabilities Services.

  7. Checks and responds to email and voicemail communication.

  8. Travels to and from customer homes approximately 20% of the time.

  9. Completes all other duties as assigned.

Minimum Qualifications

    Resident of the state of Wisconsin

  • Current valid driver's license and access to a reliable insured vehicle; both maintained throughout employment

  • Bachelor’s Degree required, preferred major in social services, human services or a closely related field is required

  • At least one (1) year of experience working with adults who have a physical and/or intellectual/developmental disability (ages 18 – 59) in a social service setting is required

  • Experience using a personal computer to carry out general office practices and data processing, as well as working knowledge of standard office equipment

  • Experience performing data entry in an accurate and expeditious manner (at a rate of at least 30 WPM

PREFERRED Qualifications:

  • Able to read, write, and speak both English and Spanish fluently
  • Knowledge of publicly funded long-term care programs
  • Experience with Forward Health, Midas and/or CARES software programs
  • Certified as Long-Term Care Functional Screening or completion of certification is required within 30 days of hire and must be maintained as a condition of employment

Knowledge, Skills, and Abilities:  Skills needed for success in this role include; customer focus, excellent verbal communication skills, patience, ability to write succinctly, professionalism, critical thinking, and being accountable. HSWs must have the ability to work collaboratively with other units and organizations within and throughout the county.
